Primary Care Package

• Maximum of 20 visits each year.
• This may include any combination of regular, after-hour, house calls and hospital visits as described below:
•  Maximum of 5 after-hour / weekend appointments per year based on your physician's assessment of your health urgency needs at the time of request
• Maximum of 5 house calls per year subject to your physician's determination of your status and requirements at the time of request
• Maximum of 20 office visits per year
• One complete annual wellness physical each year
• Cell phone access to your physician 24/7 for after hour emergencies
• On-line e-mail access to your physician for confidential non-emergency communications
• Same day / next day appointments
• Navigation and coordination of specialty needs, including acting as a liaison between specialists both in and out of the hospital

Health Value Package

Due to the rising cost of healthcare, there is a growing population without healthcare insurance. Patients are utilizing urgent care centers and emergency rooms as their primary care.

Local Urgent Care visits for common ailments (back pain, upper respiratory infection) costs range from $108 to $150 sans lab draws, imaging, medications, and physical therapy.

Local Emergency Room visits costs range from $300 sans physician evaluation, lab draws ($250 to $1000), imaging $100 to $2500, sans medications and physical therapy. Under SSMI’s Healtcare Value Package, the aforementioned services will be organized into a package provided by SSMI and various healthcare providers. The package will be advantageous from a financial standpoint as well as convenience to the patient and employer.
